Job Responsibilities: Provide oversight of Certified Nursing Assistants (CNA's) as directed by the Director of Nursing. Consult and coordinate with the interdisciplinary team (IDT) and healthcare professionals to assess, plan, implement, and evaluate individualized resident care plans. Responsible for managing resident care on a day-to-day basis. Monitor the chronic health conditions of residents; be familiar with reportable changes and potential causes for concern. Ensure that newly admitted residents have physician orders for immediate care. Provide nursing care that is compassionate and sensitive to residents with cognitive decline, memory loss or history of trauma. Provide support and assistance with resident personal care and activities of daily living (ADL's) as needed. Assist with the admission process; provide resident orientation as appropriate. Monitor the skin health of the resident; provide preventative skin care; administer wound treatments as ordered. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration Provides routine cares for patients Provides wound care Monitors patients condition including fluid intake and output and compose patients charts Assist ailing patients in daily necessary activities such as eating, drinking, bathing, etc. Communicate with patients family or friends to provide advice and comfort Communicating changes in patient behavior or health status to nurses, physicians, and family members Administering medication and monitor doses and patient response Change bandages and dressings as needed Assist patients with personal hygiene Collect specimens and deliver them to proper department Collect, track and maintain patient history and record any new data Ensure rooms are kept clean and sterile Medical Specialties: Geriatrics Physical Setting: Long-term care Ability to commute/relocate: Erskine, MN 56535: Reliably commute or planning to relocate before starting work (Required) Experience: We are willing to welcome new nurses to the workforce and give them the tools they need to succeed. License/Certification: Must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse in Minnesota Must possess current Cardiopulmonary Resuscitation (CPR) certification
